IMPORTANT:  As the Volunteer Agreement is a legally binding document, please note that the contract must be completed using the LEGAL name of the applicant whom completed the original Volunteer Application.  For example, if you completed the online Volunteer Application, your spouse or significant other CANNOT complete the Volunteer Agreement.
